The appointed registered migration agent, legal practitioner or exempt person completes the form; the client completes a declaration confirming the appointment or its withdrawal.

It is used when a client engages a migration agent, legal practitioner or exempt person for a particular immigration matter, with a separate form for each matter, and when that appointment or an authorised-recipient role is withdrawn.
