Form 80 - Complete this form if you are 16 years of age or older, and the Department of Home Affairs has asked you to provide personal particulars for assessment as part of the application process.
Purpose
This is a departmental information-gathering form used to assess an applicant's background and character as part of visa or citizenship processing. It provides a detailed personal history to support integrity and character checks.

Who lodges it
