Form 54 - Complete this form to provide evidence of your family composition.
Purpose
This is a short supporting form used to record details of an applicant's family members. It supplements a visa application (for example, a Visitor visa) by giving the department a picture of the applicant's immediate and extended family.

Who lodges it
The visa applicant completes and signs the form; where another person assists in completing it, that assistance is acknowledged in the declaration, and a witness also signs.
When it is used
Used as an attachment to a visa application when the department requires a consolidated summary of an applicant's family members (including half, step and adopted relatives), completed in English.
