Purpose
This form is a written agreement with the Australian Government in which a person commits to follow-up health monitoring or treatment after their visa is granted. It is typically required where a health examination indicated the need for post-arrival follow-up for conditions such as tuberculosis, hepatitis B or C, HIV, or leprosy, and it obliges the signer to attend an appointment with a health authority and undergo any treatment or investigation directed.
Who lodges it
The visa applicant who has been asked to give a health undertaking; where the applicant is a minor under 16, a parent or guardian signs on their behalf.
When it is used
Used during visa processing when a health assessment identifies a condition requiring follow-up in Australia, so that the applicant agrees to ongoing monitoring as a condition connected to their visa.
