Purpose

This is the departmental application form for a Former Resident visa (Subclass 151), a permanent visa for people with past residence or ties to Australia. It allows an eligible former resident to migrate to, or gain permanent residence in, Australia.

Who lodges it

The former-resident applicant lodges the form directly; where the application includes a child under 18 whose other parent is not migrating, additional consent documentation from those with legal custody rights is required.

When it is used

Used when a person who previously resided in or has qualifying ties to Australia applies for the Subclass 151 Former Resident visa, whether from outside Australia (as migration) or within Australia (as permanent residence), and mailed as a paper application to the designated processing address.
