Purpose

Captures information about a member of an applicant's family unit who is aged 18 or over, so the Department has details of each family member whether or not they intend to migrate.

Who lodges it

The main visa applicant, who completes a separate copy for each qualifying family member; the family member signs the required values statement where applicable.

When it is used

Used alongside a visa application when the applicant's family unit includes a child or other dependent relative aged 18 years or over, whether that person is migrating or not.
