Purpose

This form is used to request access to personal information or documents held by the Department of Home Affairs, under the Privacy Act 1988 and/or the Freedom of Information Act 1982. Requests may be processed under the Privacy Act (generally within 30 days) or under the FOI Act, including where a request is complex.

Who lodges it

The person seeking access to information. A nominated representative, such as a friend or migration agent, may make a request on someone's behalf, and a person seeking another individual's information must provide evidence of authority or that person's consent.

When it is used

Used when an individual wants to obtain copies of their own personal information or departmental documents, or another person's information with authority, outside the routine online options such as VEVO. Where processing charges are estimated to exceed AUD25.00, a deposit may be requested.
