Form 1528 - This form is to be completed by the DNA testing laboratory for each client that requires DNA testing for the Department of Home Affairs.
Purpose
This form is a formal statement in which a DNA testing laboratory attests to its accreditation, capabilities, and compliance with the requirements for collecting and testing samples used in parentage matters connected to departmental processes.
