Form 1455 - Complete this form if you are an asylum seeker and want to apply for Status Resolution Support Services.
Purpose
This form is used to apply for the Status Resolution Support Services program, a support arrangement that assists people to resolve their immigration status and transition toward mainstream community services. It is completed together with an SRSS Provider, who forwards it to the Department for assessment against the program's eligibility criteria.
