Form 1451 - Complete this form to confirm you received orientation from your Status Resolution Support Services Provider on emergency and fire safety procedures and safe gardening practices in and around the home.
Purpose
This form records a person's acknowledgement that their Status Resolution Support Services (SRSS) provider has given them an orientation on emergency and fire safety procedures and advice on safe gardening practices.
