This form records a person's acknowledgement that their Status Resolution Support Services (SRSS) provider has given them an orientation on emergency and fire safety procedures and advice on safe gardening practices.

The individual (and their family where applicable) receiving SRSS support, with the SRSS provider signing as witness.

When it is used

Used within the SRSS Programme after a provider has delivered home safety and gardening safety orientation, to document that the information was received and understood.
