About this copy

Original factual summaries describing what each official form is for and who lodges it. No form fields, layout or text are reproduced; the official PDF remains the only form.

Platform-authored navigation; no lawyer or registered migration agent reviewed this catalogue. It does not assess eligibility, points or suitability for any person.

Purpose

A standardised script and record for conducting a biometric identification test (photograph) on a minor under 15 years of age or an incapable person in immigration detention under the Migration Act 1958, delivered in the presence of a parent, guardian or independent person. It documents that the required information was given and understood.

Who lodges it

Administered and completed by the authorised officer conducting the test and the assisting officer, with a parent, guardian or independent person representing the detainee; it is attached to the detainee's file rather than lodged by an applicant.

When it is used

Used when a minor under 15 or an incapable person in immigration detention undergoes a biometric identification test, so the mandatory information is provided to their representative and the process is recorded.
