About this copy

Original factual summaries describing what each official form is for and who lodges it. No form fields, layout or text are reproduced; the official PDF remains the only form.

Platform-authored navigation; no lawyer or registered migration agent reviewed this catalogue. It does not assess eligibility, points or suitability for any person.

Purpose

Explains the role of an independent observer at a biometric identification test conducted under the Migration Act 1958 and records that person's agreement to act in that role. The independent person observes the test and may later be asked to recount what occurred.

Who lodges it

Completed and signed by the independent person nominated to observe the test (not the person undergoing the test), at the request of the individual required to provide biometric identifiers.

When it is used

Used when a person required to undergo an identification test asks that an independent person be present to observe, and that observer needs to acknowledge and accept the role before the test.
