Purpose
Requests a refund of a paid Visa Application Charge or related fee in the limited circumstances where a refund may be available. It captures the reasons for the request and the details needed to return an approved amount.
Who lodges it
Generally the person named as the card, BPay or PayPal account holder on the original receipt (the payer); where the payer and the person receiving the refund differ, both complete and physically sign the relevant sections.
When it is used
Used after a fee has been paid when the applicant or payer believes one of the recognised refund circumstances applies and wishes to have the amount assessed for return.
