Form 1392 - Complete this form which is a health undertaking for pregnant visa applicants who have not had the required chest x-ray because of the pregnancy.
Purpose
This form records a formal undertaking by a pregnant visa applicant to complete a required chest x-ray tuberculosis screening after the birth of their child, when the x-ray was deferred because of the pregnancy. It is an agreement to attend a Migration Medical Services Provider and follow any directed treatment.
