Purpose

This form records a formal undertaking by a pregnant visa applicant to complete a required chest x-ray tuberculosis screening after the birth of their child, when the x-ray was deferred because of the pregnancy. It is an agreement to attend a Migration Medical Services Provider and follow any directed treatment.

Who lodges it

The pregnant visa applicant who has not undergone the required chest x-ray as part of their health examinations.

When it is used

Used when a visa applicant is pregnant and the required chest x-ray has been postponed, committing the applicant to attend for screening within a set period after the visa is granted and the child is born.
