Form 1258 - Complete this form so the Department can consider care arrangements or assess the status of a minor within the Unaccompanied Humanitarian Minors Program.
Purpose
This form supports the Department in considering care arrangements for a minor under the Unaccompanied Humanitarian Minor Program and helps assess the minor's status under the Immigration (Guardianship of Children) Act 1946. It is expressly not evidence of an established care arrangement and is not a legal document.
