Purpose
This form supports the Department in considering care arrangements for a minor under the Unaccompanied Humanitarian Minor Program and helps assess the minor's status under the Immigration (Guardianship of Children) Act 1946. The proposed carer - a person over 21 who intends to provide care for the minor - completes and signs the care undertaking.
