Purpose
This form is used to request a Certification of Immigration Status from the Department confirming that a person's Temporary Resident visa has ceased to be in effect and that they have left Australia, for use in making a Departing Australia Superannuation Payment (DASP) claim.
Who lodges it
The former temporary resident seeking the certification. Each person requesting a certification completes a separate form, and a fee applies to each request.
When it is used
Used when a former holder of a Temporary Resident visa is lodging a paper DASP claim (generally where the superannuation benefit is AUD5,000 or more) and the superannuation fund requires official certification that the visa has ceased and the person has departed Australia.
